This position will be 40-50% nurse navigator, with the other 50% of time spent in 1 : 1 provider and clinic coverage.\n\nPay Range\n\n$32.45 - $48.70\n\nCabarrus Transition Services is a unique care model that is patient focused with the goal of preventing hospital readmissions. Our clinic has an interdisciplinary team comprised of Internal Medicine Providers, Cardiology Heart Failure APP, Respiratory Therapist, Nurses, CMA's, Social Worker and PharmD's. We care for patients at high risk for hospital readmission for approximately 30 days after discharge. We provide a variety of services to meet SDOH needs and / or stabilize and manage chronic health conditions. Our office is located in the Medical Arts Building on the campus of Atrium Health Cabarrus Hospital.\n\nEssential Functions\n

• Collects data pertinent to the healthcare consumer's health or the situation.\n

Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English in good, understandable terms. Intact sense of sight, hearing, smell, and touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient and / or unit conditions. Physical Abilities Testing required.\n\nEducation, Experience And Certifications.\n\nGraduate from an accredited School of Nursing; BSN preferred. BLS required per policy guidelines.
