Overview : Community Rehab Hospital West is seeking a dedicated and compassionate Travel Licensed Practical Nurse (LPN) to join our team for a day shift assignment. This role offers the opportunity to work in a dynamic rehabilitation setting, providing high-quality care to patients recovering from serious injuries, surgeries, and medical conditions.

Responsibilities :

Deliver safe, effective, and patient-centered nursing care under the supervision of a Registered Nurse

Administer medications and treatments as prescribed

Monitor patient progress and report changes to the healthcare team

Assist with patient mobility, hygiene, and daily activities

Document care accurately and timely in the electronic medical record

Collaborate with interdisciplinary team members to support patient recovery goals

Maintain compliance with hospital policies, procedures, and regulatory standards

Qualifications :

Current and valid LPN license in the state of Indiana

Minimum of 1 year of recent LPN experience, preferably in a rehab or acute care setting

BLS certification (required)

Strong communication and organizational skills

Ability to adapt quickly in a fast-paced environment
