Job Title : Occupational Therapist

Experience : New graduates are welcome. SNF experience preferred.

License : State license verification required.

Certifications : 8 hours of dementia training.

Must-Have :

COVID test completed 72 hours prior to the start date, with results issued 48 hours prior to the start date.

Description :

Ensure the prescreen form is filled out and attached.

A resume and license verification also need to be attached.

If these items are not completed and attached, the candidate will be declined.

County criminal check for the last 7 years for all counties lived and worked.

Federal criminal check for the last 7 years or more.

1 step PPD or Quantiferon or proof of positive PPD and chest X-ray required.
