Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
We are seeking Clinical Pharmacists to join a respected hospital in State College, PA . This is an excellent opportunity for a pharmacist to grow as a clinician by working across multiple units rather than being limited to one area.
Schedule :
Full-time, permanent role
Primarily day shifts (8-hour shifts)
Variable days / afternoons as needed
Setting :
Inpatient, unit-based clinical pharmacy
Focus on the Emergency Department (ED) and Intensive Care Unit (ICU / CCU)
Participation in the hospitals Antibiotic Stewardship Team
Responsibilities :
Perform order entry, medication verification, and profile review for appropriateness
Provide clinical services as needed per patient population
Participate in multidisciplinary rounds and patient care teams
Conduct drug therapy monitoring and pharmacist-managed consults per protocol
Participate in acute / emergency patient care scenarios (stroke, cardiac arrest, trauma, sepsis, etc.)
Deliver drug information and clinical support to healthcare providers
Requirements :
BS in Pharmacy or Pharm.D. degree from an ACPE-accredited school
Active Pennsylvania Pharmacist License or eligibility to obtain (out-of-state candidates wele)
pletion of PGY1 residency OR 3+ years of current inpatient clinical experience
Technology : Meditech EMR system
