POSITION SUMMARY : The Community Health Nurse will play a vital role in advancing the mission of Wayne County Healthy Communities by delivering culturally competent, patient-centered nursing care in both clinical and community-based settings. This position will oversee Medical Assistants (MAs), support workflow optimization, and provide direct patient care, chronic disease management, preventive health education, and care coordination. The nurse will also partner with Community Health Workers and other team members to improve health outcomes and address social determinants of health for underserved populations.
REQUIREMENTS :
Clinical Care & Patient Education :
- Provide health assessments, screenings, immunizations, and basic clinical services.
- Support chronic disease management (e.g., diabetes, hypertension, asthma) through education, self-management tools, and care coordination.
- Deliver individual and group health education sessions to promote wellness and prevention.
Oversight of Medical Assistants (MAs) :
Supervise and support daily MA workflows to ensure efficient patient flow and quality clinical care.Provide training, coaching, and competency checks for MAs.Monitor adherence to clinical protocols, infection control, and documentation standards.Serve as a resource and escalation point for MAs in patient care and clinic operations.Oversee clinic setup, troubleshoot issues.Care Coordination & Outreach :
Collaborate with providers, CHWs, and social service partners to connect patients to needed medical, behavioral health, and social services.Assist in follow-up of referrals and care plans to ensure continuity of care.Participate in outreach events, home visits, and community-based initiatives to engage hard-to-reach populations.Quality & Compliance :
Input results, validate documentation, flag abnormal results in software.Ensure staff compliance with FQHC, AAAHC, HRSA standards.Monitor and track health outcomes, patient satisfaction, and program impact.Team Collaboration :
Work closely with multidisciplinary staff including providers, CHWs, behavioral health clinicians, interns, and volunteers.Lead nursing / MA team huddles and participate in case conferences and quality improvement initiatives.Mentor CHWs, students, and interns in clinical practices and patient engagement.Community Engagement :
Support Neighborhood Wellness Center initiatives such as health fairs, workshops, and wellness programs.Build strong relationships with local partners, schools, faith-based organizations, and community leaders to enhance outreach and engagement.TRAVEL & FLEXIBILITY :
Demonstrate flexibility by traveling between clinic sites to provide coverage as neededAble to travel to required trainingSKILLS :
Strong patient assessment skills required including ability to triage patients. Ability to use the nursing process to guide patient encounters and care.Communicate effectively with patients in a manner that protects their confidentiality and is sensitive to their culture and to their physical / emotional / spiritual condition.Flexibility to adapt to changing or stressful conditions, including unanticipated changes to working schedules or locations.Good physical stamina and an ability to be standing / active / in motion most of the day. Strength and dexterity to move patients, handle equipment, and move quickly within the clinical area to fulfill job responsibilities.Good interpersonal skills are a necessity, including an ability to work well with the variety of ages, cultures, and temperaments represented among staff and patients, treating others with kindness and professionalism in all they do.Commitment to demonstrating personal integrity through punctuality, honesty, an ability to follow instructions,Proper attention to detail in all work matters, and a willingness learn from others.An ability to work independently, take initiative, and set priorities in accordance with the needs and mission of the clinic.Clear and concise written and verbal communication skills for communicating coherently and professionally with patients and co-workers.Conscientious of departmental and organizational policies and procedures, and able to embrace and personify the mission of the Wayne County Healthy Communities.Strong ability to be a clinical leader in the organization and guide a team of medical support personnel.Ability to delegate appropriate tasks to medical support staff.JOB QUALIFICATIONS :

Bachelor's in nursing required (master's in nursing preferred)RN License to practice in Michigan requiredClinical experience preferred (desirably in a clinic, acute care, or triage setting), but new graduates with an interest in community health are encouraged to applySalary : $36.05 hourly
