Bilingual Care Advisor

A company is looking for a Bilingual Care Advisor, Care Concierge (English / Spanish).

Key Responsibilities

Assist members with assessments and recommend service plans based on their needs

Coordinate and schedule services while making follow-up calls to members

Support members in accessing community resources such as homecare, meals, and transportation

Required Qualifications

Fluency in English and Spanish

High School Diploma preferred

Experience in Public / Community Health or related fields preferred

Proficiency in computer and clerical functions

Must be 18 years of age or older
