job_description.job_card.job_descriptionPerforming physical assessments, including vital checks. Ensuring home safety with medical equipment and supplies. Filling weekly medication planners. Overseeing skilled and non-skilled caregivers in the client's home. Graduate of a state-approved school of professional registered nursing. BSN preferred. Current, unrestricted RN license in the state(s) of practice. Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field. One (1) year of supervisory and / or case management experience preferred. Current CPR certification. Provide direct care and case management for assigned clients. Collaborate with the regional director and other personnel to enhance services. Plan, implement, and evaluate care provided. Coordinate and manage client care conferences as needed. Serve as a local on-site clinical resource to meet clients' home care needs. Assist with staffing coordination and scheduling. Ensure adherence to agency policies and nursing delegation guidelines. Participate in performance improvement initiatives. Engage in marketing activities to promote agency programs. Perform additional duties as necessary. We provide compassionate in-home care to former Nuclear Weapons Workers who are facing chronic and terminal illnesses due to their work environment. As an RN-Case Manager, you will play a vital role in enhancing the quality of life for these individuals.
Your Role :
As an RN-Case Manager, you will conduct in-home nursing visits for our clients, who are former Nuclear Weapons Workers. Your responsibilities will include :
