Key Responsibilities
Serve as the point of contact for managing student support cases across academic, social-emotional, and behavioral domains.
Coordinate services with counselors, social workers, school psychologists, and outside agencies.
Participate in Student Success Team (SST), IEP, and 504 meetings as needed.
Conduct needs assessments with students and families; maintain accurate case documentation.
Develop and monitor individualized service plans and intervention strategies.
Facilitate communication between home and school to support student success.
Provide crisis intervention and referral services when necessary.
Track outcomes and maintain confidentiality in accordance with FERPA and HIPAA regulations.
Qualifications
Bachelors degree in Social Work, Counseling, Psychology, or related field (Masters preferred).
At least 12 years of case management or student support experience (school-based preferred).
Bilingual in English and Spanish strongly preferred.
Familiarity with Santa Ana's community resources and public service systems.
Strong interpersonal, communication, and problem-solving skills.
