In 1964, we were established by a dedicated group of concerned citizens and government officials of Houston, Wabasha, and Winona counties. In 1965, we opened our first clinic in Winona. Today we provide services in Winona, Houston, Wabasha, Goodhue and Fillmore counties.

TITLE : Housing Nurse Care Coordinator
PROGRAM : Housing Services
JOB SUMMARY :
The focus will be on supporting our housing program (Winona) and psychiatric department needs (Red Wing / Wabasha) for clients with SPMI / SMI.
JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS :
Participate with other health care providers in the development and modification of a plan of care.
appropriate conferences and in-service training sessions as necessary to meet licensure and program requirements.
PHYSICAL REQUIREMENTS FOR POSITION : Must be able to move in a manner conducive to the execution of daily activities. While performing the duties of this job, the employee must communicate with others and exchange information. The employee regularly operates equipment (listed below) on a daily basis. Occasional bending and lifting of office materials may be required.
NON-ESSENTIAL FUNCTIONS : Perform other duties as assigned by the Program Director.
EQUIPMENT USED : Including but not limited to : Telephone / cell phone, vehicle, computer, scanner, blood pressure cuff, thermometer, stethoscope, scale.
JOB QUALIFICATIONS AND REQUIREMENTS :
Must possess a valid drivers license and a willingness to travel as needed to client or organization locations throughout SE MN.
Must hold a current certificate in CPR / AED / First Aid or be willing to obtain.
