About The Position

Job Title : Speech and Language Pathologist

Profession : Speech and Language Pathology

Hours per Shift : 8 hours

Experience : 1 year of SLP experience required

License : Missouri State License required

Certifications : CCC certified SLP required

Must-Have :

• LTC or SNF experience required
• 3 years experience holding active CCS preferred
• One reference at the time of submission required

Description :

Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders.

Develops appropriate plans of care following all regulatory and clinical practice standards.

Utilizes standardized assessments and evidence-based practices to support clinical interventions.

Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.

Evaluates treatment outcomes for effectiveness.

Modifies services in relation to patient evaluation and makes referrals based on outcomes.

Provides consultation and counseling to patients, families, caregivers, and other service providers regarding speech disorders.

Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.

Provides comprehensive discharge summaries of services rendered in compliance with regulatory and clinical requirements.

Documents all required information in the electronic documentation system and maintains patient records according to company policy.

Accurately records treatment minutes for services provided per all regulatory and clinical requirements.

Participates in required meetings including but not limited to patient care conferences and rehabilitation meetings.

Engages in facility and company-required training and in-service events.

Able to procure any necessary DME or supplies compliant with regulatory and clinical practice requirements.

Supervises and supports CFY or Student Speech-Language Pathologists.

Adheres to relevant practice acts, codes of conduct, and ethics during professional practice as a Speech-Language Pathologist.
