• 24 months of role experience is required with some in the last 12 months.\n
• Must have at least 24 months of Case Management experience within the past 4 years.\n
• Experience with Epic is required from any number of months.\n
• BSN completion preferred\n\nRequirements\n
• Candidates must have a Mississippi license or compact license (required after submission).\n
• This role may require floating to additional units and locations Local & travel allowed. Candidates living <50 miles from facility will be processed as local.\n\nPre-employment modules may be required for this role. • Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience.\n
• Experience must include at least 6 mos. at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge\n\nPreferred Credentials : Would consider ADN\n\nUnit Details : Will cover all Inpatient units throughout the hospital.\n\nShift & Scheduling\n
