Our Nurse Practitioner (NP) is responsible for periodic and episodic assessment and management of individual participants medical conditions, and for oversight of their use of medical specialists and inpatient care. The NP is part of an Interdisciplinary Team with the knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants.Required Education :

Completion of an accredited, Board-approved masters, postmasters or doctoral nurse practitioner program, current, unrestricted license as a Registered Nurse in this Pennsylvania, and current National certification as an Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP). Required Licensure :  Pennsylvania RN license, Master level, post Masters program, or Doctoral degree from accredited Nurse Practitioner program. Active CRNP Certificate in Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP), active Pennsylvania Prescriptive Authority Approval and a DEA license.
