Role Description

This is a full-time / part-time / per diem role for a Home Health Nurse in McHenry county IL. The Home Health Nurse will provide in-home care to patients, including administering medication, monitoring patient health, and providing wound care. Day-to-day tasks will include developing and following treatment plans, assessing patient conditions, educating patients and families on proper care techniques, and collaborating with other healthcare professionals to ensure comprehensive patient care.

Qualifications

• Registered Nurse (RN) license in the state of Illinois required
• Experience in Home Health and Patient Care strongly preferred
• Proficiency in Wound Care
• Strong communication and interpersonal skills
• Ability to work independently and in a team environment
• Reliable transportation and flexibility to travel for home visits
