Job Description

• This is a Part Time position at 20 hours per week located at Lake Forest Hospital. It is an inpatient role focused on working in collaboration with our palliative physicians and advanced practice providers.  Schedule is as follows : Wednesdays, Thursdays and every other Friday 8 : 00am-4 : 30pm.

The Social Worker reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

The Social Worker is responsible for providing psychosocial counseling and intervention services to patients who request their services, or any patient referred by a provider. The social worker advocates for the patient and family, and assists in outpatient continuity of care.
