What You'll Do

The Licensed Practical Nurse (LPN) works collaboratively with the care team to provide ongoing support and communication to chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD) patients. The LPN acts as a single point-of-contact to coordinate resources along the care delivery spectrum, identify gaps, and provide proactive follow-up. The LPN is responsible for making sure the patient's care at various locations is connected and there are no gaps in care or communication. This role will report to the Sr. Manager, Market Operations.

The Day to Day

Minimum Qualifications

5+ years combined of related education, experience, or certification.

Active LPN (Licensed Practical Nurse) Licensure in state of practice.

Efficient and reliable transportation, including an active driver's license, allowing for the ability to travel across an assigned region to meet patient needs. Locations may include offices, clinics, and patient homes.

Preferred Qualifications

Hourly Salary Range : $28.00 - $32.25
