We are currently seeking an experienced :
MEMBER NAVIGATOR
JOB SUMMARY :
- Assists with access to care for CSNP members in collaboration with RN case manager and nurse practitioner.
- Obtains information for members based on the model of care requirements.
- Obtains information to assist with evaluation of patient needs.
- Communicate effectively with individuals across the care continuum.
- Documents member information in the electronic medical record.
- Reviews appointments communication pertinent information to RN Case Manager(s) and nurse practitioner(s) for additional interventions
- Collaborate with IDT for coordination of care
- Communicate with member and / or family obtaining and providing information regarding care and needs.
- Strong communication skills and focus on customer needs and services.
- Assists with any patient emergency as needed.
- Recommends and / or conducts specific areas of in-service or continuing education with patients and caregiver as appropriate.
- Assists with compliance of quality measures for CSNP program
- Assist with HEDIS measures and collection of data
- Assist with collection and presentation of QA data for the CSNP program
- Participates on ICT meetings for CSNP members
- Functions in CNA role if needed for member visits
- Performs other duties as assigned.
JOB REQUIREMENTS :
Graduated from an accredited Certified Nursing Assistant (CNA) with a current CNA certification in the state of practice preferred2 years’ clinical experience as a Certified Nursing Assistant (CNA) in geriatric, adult or family practice setting, preferred2 years’ experience working in a community-based program involving geriatric, adult or family practice settingPertinent clinical work experience relevant to population served.Current BLS certificationMust be able to demonstrate computer skills, including use of e-mail and Microsoft office word and excelMust be able to demonstrate effective written communication skills including but not limited to writing reports and correspondenceMust be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to members, providers, families and staff and listening to members, providers and familiesDemonstrated ability to meet deadlines, and use time effectively and efficiently.Demonstrated successful experience working in an environment of change. Effectively copes with change and uncertainty; responds and "shifts gears" comfortablyIs dedicated to meeting the expectations and requirements of both external and internal customers, acts with customers in mindDemonstrated ability to establish and maintain effective relationships with customers, and gain their trust and respectUses logical methods to solve problems with effective solutions; looks beyond the obvious for solutionsIs action oriented and sees challenges as opportunitiesKnowledge of community resourcesPrevious experience working with behavioral health populationsExperience working with low income and disadvantaged populationsPrevious work with vulnerable adults or senior populationMust meet company driving standardsMust have valid state driver’s license and be able to drive a carMust meet company's automobile liability insurance standards and provide proof of coverageMust be able to independently travel to assigned locationsVisit www.libertycareers.com for more information.
