SSN Address Trace required.Competency Exam Completed within one (1) year of start date for the specialty corresponding to the CP s Assignment; required annually. All tests must have an 80% minimum or passing angoff score.Driver's License or Government ID Valid Driver's License or Government issued IDDrug Screen 10-panel drug screen collected within sixty (60) days of start date; required annually.Hepatitis B Shown by proof of vaccination series, positive surface antibody series or declinationInfluenza For Assignments occurring between 10 / 1-3 / 31. Must be run for all alias names that appear on the SSN address trace.Physical Completed within one (1) year of start date; required annually. Must be signed by MD, DO, DC, NP, or PA.SAM (System for Award Management) SAM completed within thirty (30) days of start date, and at time of extension. Must be run for all alias names that appear on the SSN address trace.Verification of Professional License for State of Maine Online verification of professional license for the State of Maine completed within thirty (30) days of startVaricella Shown by proof of two (2) vaccinations, positive IGG titer, or provider verified historyTdap Administered within ten (10) years of start dateTB Documentation of two (2) TB skin tests, one within a year of start date and the other TB test within thirty (30) days of start date. TB skin test should be repeated annually thereafter. TB QuantiFeron completed within one (1) year of start date and annually thereafter would also be acceptable. Chest X-Ray should only be completed for past positive PPD cases as a result of TB exposure. Chest X-Ray result must indicate that the Chest X-Ray was completed to rule out positive TB results. The Chest X-Ray results are good for 5 years. If providing Chest X-Ray, past positive TB result and TB questionnaire is required at time of placement. Requirements :   Licensed as a Physical Therapist in the State of Maine and active BLS issued through American Heart AssociationShift :   Will consider 8 hour days; 8 : 00AM-5 : 00PM or 10 hour days; 7 : 00AM-6 : 00PM with an hour lunch.  Orientation :   Will start on a Monday at 7 : 45am to coincide with orientation at Northern Maine Medical Center.  Orientations occur on Monday every other week unless Monday is a recognized holiday. Guaranteed Hours : 40 Contract Weeks : 89
