The Certified Diabetes Educator (CDE) possesses clinical expertise in diabetes management and other chronic diseases.

THINGS YOU MAY WANT TO KNOW ABOUT :

Location : Sandy, Utah (around 7400 South and 1300 East). Plenty of free parking. There are numerous restaurants and shopping options nearby.

Schedule : Monday through Friday. The start and end times are to be determined. Once set, they will stay the same. The Clinic is open Monday through Friday, 8 am to 5 pm.

ROLES & RESPONSIBILITIES

Provide diabetes and self-management guidance and education to patients with prediabetes, type 1, type 2, and gestational diabetes.

Develop individualized diabetes education plans for patients collaborating with medical providers and monitor adherence to the care plan. Individualized education plans should consider different cultural, physical, cognitive, and literacy levels of patients and families and meet evolving needs of patients.

Provide nutritional counseling for weight and diabetes management. Use comprehensive knowledge of nutrition therapy to assist patients with meal planning.

Teach patients to manage blood glucose levels and dose insulin using carbohydrate counting.

Collaborate with patients and medical providers to develop and monitor the progress of a physical activity plan that accommodates patients needs, abilities, and self-management goals.

Assist medical providers in assessing the need for, and frequency of, self-monitoring of glucose levels. Teach patients how to self-monitor glucose levels using various diabetes technologies. Monitor and evaluate patient glucose logs and CGM reports.

Instruct patients and family members on diabetes-related medications including basic mechanism of action, administration, and safety. Additionally, educate patients and family members on the prevention, recognition, and treatment of hypo- and hyperglycemia. Teach patients how to administer insulin and other injectable diabetes medications. Assist medical providers with medication doses according to glucose patterns, dietary habits, and individual barriers to care.

Work with patients and the diabetes care team to ensure that patients have access to medications and diabetes-related supplies. Also, it helps to ensure that patients keep appointments with members of the diabetes care team.

Identify individual social and emotional barriers to compliance with the treatment plan. Refer patients to mental health professionals and resources as needed.

Facilitate care coordination with the diabetes care team, pharmacies, and durable medical equipment suppliers.

Assist in the review and / or development of clinical practice guidelines. Serve as a content expert for diabetes education and management.

Assist in developing policies and procedures for diabetes and health education guidelines.

Support the organization as needed.

Provide diabetes education and device training for clinical research studies.

Oversee, submit documentation for, and monitor TZIELD candidates.

REQUIRED QUALIFICATIONS & EXPERIENCE

Bachelor of Science in Dietetics from an accredited facility

Licensed in Utah as a Registered Dietitian

Certification by the National Certification Board for Diabetes Care and Education is preferred.

At least 2 years of experience caring for individuals with diabetes is preferred.

Professional knowledge and skill as a Diabetes and Health Educator

Ability to investigate and analyze information, as well as drawn conclusions

In-depth knowledge and understanding of the nature of diabetes and related issues, as well as patient care concerns

Ability to design, develop, deliver, and evaluate educational programs, classes, and materials

Ability to provide diabetes-related technical advice, guidance, and support to patients and members of the diabetes care team

Strong interpersonal and communication skills

Knowledge of the cultural, social, and economic background of various patient populations served including the impact of cultural values and beliefs in their diabetes status. Ability to establish rapport and gain the confidence of patients and familiar, as well as members of the diabetes care team

Ability to work independently and take the initiative in addressing concerns and issues

Proficiency with computer-based applications such as electronic health records, HIPAA compliant patient communication platforms, Microsoft Office, Google Workspace, etc.
