Job Title : Certified Occupational Therapy Assistant

Experience : New Graduates are acceptable; SNF experience preferred.

License : State License Verification required.

Certifications : CPR (fundamental facilities) required.

Must-Have :

• Completed prescreen form attached upon submission.
• Resume and license verification attached upon submission.
• References (3) provided.
• Negative drug screen results required.
• Covid test completed 72 hours prior to the start date, with results issued 48 hours prior.
• 1 Step PPD or Quantiferon proof, or proof of positive PPD and chest X-ray required.
• Criminal background checks for county, federal, multi-state (7 years or more) and nationwide sex offender search (7 years or more) required.
• HHS / OIG search and GSA search required.
• Social security number trace required.

Description :

References are required from three individuals.

Criminal background checks will be performed for all counties lived and worked in for the past seven years.

A federal criminal background check will also be conducted for a minimum of seven years.

A multi-state criminal search and a nationwide sex offender search for seven years or more will be required.

HHS / OIG and GSA searches will be conducted as part of the credentialing process.

A social security number trace must be provided.

Candidates will need to provide proof of a negative drug screen.

Compliance with the Covid test requirement is essential prior to the start date.

Candidates must meet all requirements to be considered for the position.
