What You'll Do
Participate in Interdisciplinary Team (IDT) meetings to assess needs and develop personalized care plans.Evaluate functional abilities, strength, and mobility to design targeted therapy programs.Deliver therapeutic services including gait training, strength conditioning, and mobility restoration.Facilitate engaging individual and group exercises focused on rehabilitation and wellness.Provide care aligned with physician orders and individual treatment goals.Educate participants and their families to support safe and effective home recovery.Recommend follow-up care and referrals based on patient progress and needs.Conduct occasional in-home or facility-based therapy visits when necessary.Collaborate with nursing homes, assisted living, and SNF staff to oversee participants’ rehab journey.Maintain and manage therapy equipment for safe and efficient operations.Schedule & Work Environment
Hours : Monday–Friday, 8 : 00 AM – 5 : 00 PM (100% on-site)Travel : Minimal; occasional local travel to homes or care facilitiesPerks & Benefits
