- Handle triage of after-hours patient and family phone calls, assessing needs for services such as assessments, symptom management, and care plan updates, while delegating nursing visits to appropriate staff\n
- Gather information on physical, psychological, social, and spiritual aspects affecting patient and family needs, initiating interventions with the interdisciplinary team as necessary\n
- Communicate effectively with attending physicians and interdisciplinary staff to coordinate optimal patient care\n
- Perform clinical pronouncements of death and offer family support\n
- Maintain accurate clinical records with timely care plans and progress notes\n\nQualifications : \n
- Graduation from an accredited registered nursing program\n
- Current registered nurse license to practice\n
- Minimum of two years of diverse nursing experience\n
- Proficient in nursing practices and clinical judgments\n
- Ability to manage emotional stress among families with respect for individual lifestyles\n
- Current CPR certification required\n
