Endovascular

  • Bypass Surgery
  • Femoral Popliteal Bypass
  • Aortofemoral Bypass
  • Stenting
  • Endovascular and Open Surgical Reconstruction for Deep Vein Occlusions

    • Treatment of

  • Vascular Malformations
  • Nutcracker Syndromes
  • Lymphedema
  • Gangrene of the limbs
  • Peripheral Artery Disease

    • SUBMISSION REQUIREMENTS

  • Board Certified Vascular Surgeon - REQUIRED
  • Fellowship Training - REQUIRED
  • OH License - Preferred , willing to accept providers needing to obtain OH license ( Note status at time of name clear)
  • BLS & ACLS Certified - REQUIRED
  • COVID Vaccine - REQUIRED , noted at time of name clear

    The provider will be covering 4 locations

    Practice Setting : Hospital, Will not require clinic hours

    Average Number of Cases Per Day : Rounding at 4 hospitals with an average of 6 patients at each hospital

    Documentation System / EMR : EPIC

    Trauma Level / Designation : 1

    Number of Operating Rooms : Each hospital has multiple

    Patient Age Demographic : Adult

    Minimum Board Certification : BC

    EMR System : Epic
