Senior UAS Assessor Registered Nurse (RN)
Location : Bronx, NY (Field-Based, Hybrid)
Pay Range : $35.49 – $63.79 per hour + 8% Annual Bonus Potential
Schedule : Monday to Friday, 8 : 30am – 5 : 00pm EST

We are seeking experienced RNs to join our Long-Term Services & Supports (LTSS) team as fieldbased Assessors. In this role, you’ll conduct UAS assessments and develop care plans that directly improve quality of life for members with complex and high-acuity needs.
What You’ll Do
- Conduct UAS assessments (2–4 daily) for members in the Bronx to evaluate needs and eligibility
- Develop and update personalized care / service plans for members with medical or physical health needs
- Educate members and caregivers on benefits, resources, and care options
- Collaborate with providers, specialists, and care teams to ensure effective, person-centered care
- Monitor and document member progress, conditions, and care goals in compliance with regulations
- Serve as a trusted liaison and advocate between members, families, and providers
- Partner with leadership to continuously improve quality of care and outcomes
