Description : About PACE North
PACE North (Program of All-Inclusive Care for the Elderly) helps adults 55+ who qualify for nursing facility care remain independent and safe in their homes while receiving the support they need. Our team provides care both at our state-of-the-art day center on Garfield near Hammond, and in participants’ homes throughout the community. The RN Care Manager Role
As an RN Care Manager at PACE North, you’ll be more than a nurse - you’ll be a decision maker, advocate, and trusted partner in care. In this role, you will :
Serve as a core member of the interdisciplinary team, shaping care plans and ensuring participants’ needs are met.Collaborate with staff, participants, and families to build strong, supportive relationships.Conduct initial and periodic health assessments.Monitor and coordinate supplies, equipment, and services.Visit participant homes and participate in the on-call rotation.Lead care transitions, including hospital-to-home discharge planning.Provide education, advocacy, and leadership that empowers participants to live their best lives.This is a full-time, in-person position (not remote).

What We’re Looking For
