Job Summary :

The coordinator is responsible for overseeing the management of all CHAP client data, which includes gathering, preparing, updating, and maintaining client paperwork and ensuring client eligibility for services. This person will work with his / her and other Programs staff to assess client needs in order to develop, coordinate, monitor, and update all services for the client; and ensure that standard is met for the entire department. The coordinator is responsible for working with Finance and Accounting on billing consultative services, as well as investigating specific client issues associated with internal documentation and external requirements from organizations such as Medicaid or the PASSE. This person ensures clients receive all necessary services in a planned, coordinated, efficient, and effective manner.

Supervisory Responsibility : No

Essential Job functions include :

  • Accountable for the workload assigned to the CHAP Care Coordinator staff / contractors.
  • Responsible for supporting the team with planning, preparation, training, and ongoing development of the Care Coordinators as assigned.
  • Arrange for the provision of Waiver / ADDT services, including the following responsibilities :
  • Attend and assist with Individual Program Plans and Person-Centered Service Planning meetings to coordinate services in a way that objectives can be met.
  • Support CHAP Contractors to effectively plan client care in the context of operations to ensure no disruption of services.
  • Explain schedule information, client progress, health and safety concerns, etc., to complete plans of care.
  • Assist with compiling and maintaining Medicaid, PASSE, personnel enrollments / paperwork, and develop contracts during an intake and throughout the client’s time with ERC following DDS, DPSQA, and Medicaid requirements and Elizabeth Richardson Center policies and procedures.
  • Develop, compile, and maintain the prior authorizations, PCSP, behavior support plans, and other service paperwork for clients following DDS, DPSQA, and Medicaid requirements and Elizabeth Richardson Center policies and procedures
  • Assist the care coordinator team by providing directions if there are issues with Medicaid, PASSEs, or other entities.
  • Standardize the process so that ERC can capture all provided services.
  • Bill consultative services as assigned.
  • Assess the needs of the client and assist with the development of plans provided by the PASSE in collaboration with the ERC program.
  • Make referrals for other services when appropriate.
  • Advocate for clients and ensure we only provide those services we have the capability and capacity to provide.
  • Coordinate and facilitate additional interdisciplinary meetings as needed to address programming issues.

Qualifications - Knowledge / Skills / Abilities include :

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write detailed reports and correspondence. Demonstrates both verbal and written communication skills.
  • Skilled in operating computer systems and programs i.e., Microsoft Word, Outlook, and Excel, and the ability to prepare documents; interpret and develop spreadsheets.
  • Knowledge of policies, procedures, and regulations for ERC, DPSQA, Medicaid, and other affiliated client care procedures and policies required for billing and coordination of care.
  • Knowledgeable of adult learning training methods.
  • Ability to deliver a variety of training methods.
  • Ability to prepare training material and packets, as needed.
  • Ability to communicate effectively and in a timely manner with clients (and guardians), the general public, colleagues, and supervisors.
  • Ability to read and interpret documents, reports, or manuals.
  • Work Environment :

    The physical work environments may have extreme heat or cold, and loud noise levels.

    Physical Demands :

    This role requires sitting, standing, walking, lifting, carrying, reaching, pushing, pulling, repetitious computer and phone use, and talking to people, minimum weight, etc.).

  • frequently required to stand, walk, sit, lift, carry, push, turn, twist, and reach.
  • Travel : Moderate-conducting monthly home visits to CHAP Members and in-home / onsite support for other ERC programs as needed.

    Required Education and Experience : education and experience based on requirements that are job-related and consistent with business necessity .

  • Minimum level of education required – Bachelor’s degree in the human services field, or an Associate’s degree (or equivalent) with two plus years of experience in a similar role serving individuals with intellectual disabilities; five plus years of experience in a similar role may be considered instead of educational requirements.
  • Experience working with individuals with intellectual disabilities.
  • Proficiency in Microsoft Office Suite is required.
  • Additional Eligibility Qualifications : Must pass an extensive background check.

  • Must comply with drug testing requirements.
  • Valid Arkansas driver’s license
  • Reliable transportation is required.
  • Licensure requirements- Must meet minimum licensure annual training requirements.
  • This role includes licensure requirements for Administrators, Health & Safety, and HIPAA training.

  • CPR / First Aid trained and updated annually
  • Other Duties : As required to fulfill the ERC mission.
