  • Primary Source Verification of professional license, Nursys, and / or certification, as required for the position ran within 30 days of start updated upon expiration.
  • Must score 80% or higher on Prime Core Competencies Exam.
  • Must score 80% or higher on Prime Specialty Exam.

  • Valid for 2 years. Available for free at https : / / calcivilrights.ca.gov / shpt /
  • Annual Respirator fit test. Locations that allow onsite testing : NJ - Saint Michael's Medical Center, NJ-Saint Mary's Hospital (Mask Accepted : 3M 9205+), CA-West Anaheim Medical Center, CA-Garden Grove Hospital Medical Center.
  • Flu Vaccine or declination required October 1-May 31. Declinations not accepted at Shasta Regional Medical Center.
