Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract role in Pittsfield, MA. This position involves providing skilled nursing care in patients homes, completing OASIS documentation, and collaborating closely with the interdisciplinary team to ensure quality patient outcomes. Candidates should be comfortable managing a caseload, meeting documentation deadlines, and traveling between patients.

Key Responsibilities :

Provide skilled nursing care to home health patients

Complete comprehensive OASIS assessments and documentation

Develop and implement individualized care plans

Educate patients and caregivers on treatment plans and health management

Coordinate with the care team to ensure continuity of care

Qualifications :

Active RN license in MA

Minimum 2 years of nursing experience

At least 1 year of recent home health experience

Proficiency with OASIS documentation

Valid drivers license, reliable vehicle, and auto insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
