Job Summary : Wound Care Nurses are responsible for assessing and identifying patient needs and implementing and monitoring patient medical plans and treatments. Wound Care Nurses perform wound care techniques to ensure patient health and safety.
DUTIES AND RESPONSIBILITIES :
- Participates in all aspects of resident care to ensure safe, efficient and therapeutic nursing care.
- Creates and reviews care plans. Ensures care plans, medications, treatments, and nurse notes comply with physicians orders.
- Completes admission assessments. Assists in the referral, admission, and discharge process.
- Assesses patients for injuries in the event of an accident and documents and reports the incident to the provider and management.
- Monitors patients for any changes in condition and intervenes early by notifying the Director of Nursing and / or Administrator of the changes that occurred.
- Conducts and assesses residents wounds with thorough documentation of wound measurements, residents nutritional status, family updates, etc., on a weekly basis.
- Dresses and performs wound care on all type wounds as ordered by physician. Documents care in the resident's medical record.
- Performs frequent resident checks on high risk residents to ensure they are being turned according to schedule.
- Prepares and provides staff education on wound prevention including the assessment, treatment, and documentation of wounds.
- Serves as a medication nurse.
- Gives medications, including IV piggyback-diluted medications and IV push medications.
- Collects lab specimens and records results in the patient's chart. Notifies physician of lab results.
- Assists with patient care, including wound care, Activities of Daily Life care, meals, etc.
- Ensures patients are placed on infection control precautions. Communicates to other staff regarding the precautions the resident is on. Ensures PPE is readily available and set up on the patient's door.
- Charts medications and any other pertinent patient information.
- Communicates changes in patient conditions to physicians, Pavilion staff, and families. Documents changes on patient record.
- Completes unit safety checks and identifies, eliminates, and reports any identified safety issues every shift.
- Enters patient charges every shift.
- Completes patient documentation every shift.
- Participates in nurse rounding and reporting prior to shift change.
- Orders patient supplies and materials.
- Assists with 24-hour chart checks.
- Participates in Medicare, care plan, Quality Assurance and Performance Improvement, and Infection Control meetings.
- Orders, receives and distributes supplies for all halls and administration, weekly.
- Acts as a preceptor for nursing students and orients new hires of the same discipline.
- Takes call rotations.
- Identifies and helps in resolving problems within nursing service.
- Keeps the work station clean and assists with housekeeping duties(employee fridge, putting away supplies, etc).
- Performs other job-related duties as requested.
Education / Qualification Requirements :
Current RN / LPN License in the State of Georgia.Current CPR certification. (Must obtain within 90 days of hire).Current CPI certification. (Must obtain within 90 days of hire).Current ACLS certification. (Must obtain within 6 months of hire)Experience :
