Youth Housing Case Manager

Cocoon House is dedicated to eliminating youth homelessness and ensuring the safety, resilience, and holistic health of our community's young by providing targeted, supportive services. We are seeking a compassionate and dedicated Youth Housing Case Manager to join our team. This role involves direct case management services to youth aged 12-24 who are experiencing homelessness or are at risk. The ideal candidate will operate with a trauma-informed approach, emphasizing the creation and implementation of individualized support plans that encourage the attainment of personal stability and growth toward sustainable independence.
