Nationwide Therapy Group is seeking a LPN / LVN Long Term Care for a travel job in Saint Louis, Missouri.

⏰ Shift : 8 Hr Evening (2 : 45 PM – 11 : 15 PM)

🗓️ Schedule : 5x 8hr shifts per week

📅 Every Other Weekend (EOW) required

🏥 Facility Type : Nursing Home

📍 Location : Saint Louis, MO

🏠 Housing : No housing available

️ NEED : Must pick up 2-4 per diem shifts before submission OR have completed a successful assignment through GrapeTree prior to submission

Requirements : ✅ CURRENT RESUME

MUST Include :

Facility Name

Month & year for each facility

Charting systems used (ex. : EPIC, PCC, etc.)

Duties at each facility 🩺

✅ REFERENCE

1 Supervisor Reference (from last year) : Include name, facility, and year
