Description :
Utilizes the nursing process to assess patient / caregiver needs in order to provide quality care to assigned patients.
Develops and communicates a realistic plan of care for each patient.
Serves as Case Manager for assigned patients and actively manages case loads and visit times.
Effectively completes OASIS documentation in accordance with agency policies and regulations.
Implements medical plan of care, nursing interventions, and procedures as necessary.
Stays up-to-date on high-tech skills such as IV use and maintenance, wound VAC, and complex wound care.
Works collaboratively with LPNs to ensure that the plan of care is followed and patient needs are met.
Evaluates various aspects of the implemented plan of care to meet patient and caregiver needs.
Maintains accurate documentation of nursing care provided to ensure the integration of information for the healthcare team.
Follows agency protocol with processes including visit completion and follow-up notes.
Performs patient and caregiver education to ensure informed decisions can be made.
Identifies interdisciplinary needs of the patient and documents needs accordingly.
Completes various agency, state, and federal forms accurately and in a timely manner.
Carries out visits efficiently and communicates information to the scheduler promptly.
Assesses patient needs for additional services, develops and coordinates plans of care, and supervises care.
Transmits patient incident reports to the appropriate specialist in a timely manner.
Coordinates safe patient discharges and ensures all appropriate paperwork is processed.
Coordinates patient care with physicians, providers, and team members in support of the plan of care.
Participates in monthly peer review audits providing and accepting constructive feedback.
Schedules and facilitates case conferences with team members to maximize communication.
Advocates for patient care issues to ensure that overall quality of care is sensitive to specific patient and family needs.
Participates in interdisciplinary team meetings to develop an individualized plan of care within five days of admission.
Stays informed on pain control, symptom management, and high-tech skills.
Performs patient and caregiver education to ensure informed decision-making can occur.
Coordinates a variety of patient care with physicians, providers, and team members to ensure safe and effective care.
