Job Summary
Partners in Home Care is seeking a dedicated Physical Therapist to provide skilled evaluation and rehabilitation services to patients in their homes. This role includes direct care, care planning, patient / family education, and coordination with an interdisciplinary team to ensure high-quality, evidence-based outcomes.
Key Responsibilities
Evaluate patients and develop individualized care plans in the home settingProvide direct, skilled physical therapy per plan of care and physician ordersAdjust treatment frequency and duration based on patient progressComplete and score OASIS assessments; collaborate with the Quality teamMaintain knowledge of Medicare regulations, Conditions of Participation, and payer requirementsPerform physical therapy interventions, including wound care when appropriateDocument care and communication using point-of-care systemsCoordinate with schedulers, optimize travel routes, and communicate availabilityParticipate in care conferences and agency quality improvement initiativesSupervise Physical Therapist Assistants and CNAs as requiredPlan and manage admissions, re-evaluations, and dischargesMaintain licensure and continuing education standardsUphold ethical standards and participate in agency training sessionsQualifications
Bachelors degree or higher from an accredited Physical Therapy programCurrent license to practice in the State of MontanaValid drivers license, auto insurance, and reliable transportationAccess to high-speed internet2+ years of experience preferred, especially in home health or clinical rehabSchedule
Schedule may vary depending on FTE status and patient care needs, as directed by the supervisor. Typical workdays areMonday through Friday.
