Personal Care Champion
Provides basic patient care under the direction of a licensed nurse. Serves as patient's 'Personal Care Champion' by assisting them with activities of daily living. Provides care & support in the home setting or hospice house with special attention to patient personal care needs in end of life care. Perform visits and / or shift work.
What You Will Do :
- Administer treatments, such as application of prosthetics / braces, Anti-embolism hose, simple dressings, non-prescription topical creams / lotions, massages, as directed by a physician or nurse.
- Bathe, groom, shave, dress, ambulate, transfer, and perform Range of motion (ROM), exercises, empty urinary or fecal drainage bags.
- Clean rooms / home, change linens, empties trash, restocks supplies, and prepares meals or snacks and assist with feeding patients.
- Collect specimens such as urine, feces, and or sputum.
- Observe patients' conditions, measuring and recording food and liquid intake and output and vital signs, and report changes to professional staff.
- Turn and reposition bedridden patients, alone or with assistance, to prevent bedsores and educate family in repositioning.
- Answer patient call lights, phone calls and direct visitors in the hospice house.
- Completes documentation in Electronic Medical Record (EMR).
Qualifications :
H.S. Position Demands : Is on call on rotating weekends, irregular shifts and may have capability to work in the hospice house. Will drive personal and or company vehicle. Driver's license required. Hospice House Physical Demands : Will be exposed to dog visits, temperature controlled. Position Demands : Works multiple variable shifts, works weekends and may have capability to do home visits. No Driver's license required. Not expected to driver personal and or company vehicle.
