Duration : 05-06 Months + Possible extension
Schedule Notes : One of three shifts : 7am to 5 : 30pm - 8am to 6 : 30pm - 11am to 9 : 30pm
Duties :
"To promote pharmaceutical care by managing the medication process, identifying and preventing potential drug-related
problems, and resolving actual drug-related problems. Additionally, to perform the duties of a registered pharmacist as
defined by Texas Children's Hospital. All responsibilities will be performed in a manner developmentally appropriate for the

 Participates in the education of patients, caregivers, and the community
 Supports patient safety systems
 Checks all available means of communication including e-mail, mailbox, and voicemail every shift
 Ensures the smooth running of the pharmacy and acts as a role model for technicians and other pharmacists as evidenced by lack of complaints, medication incident reports, and peer review
 Maintains investigational drug records according to departmental policy
 Contribute to the medication management process
 Conduct discharge medication review (difficult to obtain, prescription accuracy) and counseling, ensuring that patients / caregivers understand their medication therapies upon discharge
 Participates in study initiation, maintenance and closure

o Bachelor’s degree or Ph.D. in Pharmacy required
o RPH certification from the state of Texas required
o 2+ years hospital staff pharmacist experience (preferred) OR 5+ years retail pharmacist experience at a high volume store
o 20 hours of training in compounding sterile products accredited by the Accreditation Council for Pharmacy Education or the American Society of Health-System Pharmacists preferred
Education :
"Required- Bachelor's Degree Pharmacy Or
Ph.D. Pharm. D. degree"
Required- RPH - Pharmacist State of Texas
"The pharmacist in this role shall be a licensed practitioner who holds a valid, unrestricted license to conduct clinical
activities."
