POSITION SUMMARY
To provide superior quality, competitive value, and outstanding service by delivering the highest standard of quality medical care to Brevard Health Alliance (BHA) patients through the primary care medical home model. This unique midlevel role will be divided between two primary functions :
- Clinical Provider Integration and Onboarding – leading the development and facilitation of clinical training and orientation for new and existing providers, and
- Community-Based Home Visits – conducting home-based primary care visits for patients with barriers to transportation, mobility challenges, or significant medical complexity. These visits will primarily occur in multi-family residential settings, such as apartment complexes, senior living communities, and similar environments.
This position requires a patient-centered mindset, strong teaching and communication skills, clinical expertise, and the ability to work independently in both clinical and community settings.
GENERAL EDUCATION REQUIREMENTS
Nurse Practitioner (NP) or Physician Assistant (PA) licensed to practice in the state of FloridaCurrent and active DEA registration (or ability to obtain upon hire)Board certification in primary care (e.g., FNP, PA-C)Current Basic Life Support (BLS) for Healthcare Providers from the American Heart AssociationADDITIONAL QUALIFICATIONS
Minimum 2 years of experience as a midlevel provider in a primary care settingExperience working in community-based care or home visits preferredExperience in a Federally Qualified Health Center (FQHC) or similar underserved settingFamiliarity with quality metrics (e.g., HEDIS, UDS) and population health strategiesStrong understanding of electronic medical records; eClinicalWorks (eCW) experience highly preferredComfort with Microsoft Office Suite and learning management systemsPrior experience in provider education or clinical training preferredAbility to facilitate educational sessions for individuals and groups, both in-person and virtuallyPRIMARY ACCOUNTABILITIES
Community-Based Patient Care (80%)
Conduct home visits for patients facing transportation or mobility challenges, focusing on chronic condition management, preventive care, and complex medical needsDeliver compassionate, culturally sensitive care in residential settings, with a focus on care coordination and reducing barriers to accessCollaborate closely with care teams, case managers, and external partners to ensure comprehensive support for patients seen at homeDocument visits and coordinate follow-up within the EMR and with the primary care teamIdentify gaps in care and help connect patients to in-clinic or specialty services when neededMaintain visit schedules, route planning, and safety protocols for community-based careClinic-based patient care and provider coverage may also be routinely required based on patient volume and community needsTraining and Onboarding (20%)
Lead the onboarding, orientation, and EMR (eCW) training for new providersDevelop and update clinical training materials, including guides, workflows, and multimedia resourcesProvide individualized and group-based training in EMR use, clinical workflows, and quality initiativesCollaborate with HR, Clinical, Credentialing, and Quality to ensure training is aligned with organizational requirements and standardsServe as the Clinical eCW Superuser and support provider adoption of documentation and workflow standardsSupport the implementation of clinical quality and performance improvement initiatives through provider education and engagementEnsure all accesses are complete prior to new providers entering patient care (eCW, Tokens, Maven, Azara, UpToDate, etc)
