Case Manager-Shelter

POSITION SUMMARY :
In this role, Case Manager-Shelter will provide intervention and case management services to assist domestic / sexual and other forms of violence survivors in obtaining permanent housing and supportive services during their experience of homelessness, the eviction process, and housing stabilization. This position will team advocates, and care coordinators to provide crisis intervention, advocacy, referral, coordination, and other case management and supportive services related to housing / economic / well -being and stability for survivors at the Agency’s main shelter.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
- Assist survivors in locating and retaining housing while addressing the immediate needs of the client / household including emotional support, promotion of healthy relationships and housing that supports safety and security.
- Provide referrals to support services designed to stabilize households in permanent, habitable housing including services related to substance use disorder, financial security, public benefits, education, employment, parenting, mental health and physical well-being.
- Advocate and collaborate with landlords, housing programs, and department of social services.
- Identify and integrate the needs related to the dynamics of domestic and sexual violence, trauma impact, and crisis into the support and management of the resident.
- Provide and support on call (after hours / weekend) needs on a scheduled rotation for Vera House shelter.
- Maintain resident documentation using software databases.
- Answer and provide assistance with support-line services, as needed.
- Work closely with staff to obtain feedback and input from clients as well as ensure adequate services for clients in the program, including if the client feels unsafe and needs to be transferred.
- Maintain strong professional boundaries, limited to the extent of professional relationships, ensuring compliance, ethical and appropriate interactions.
- Assist with additional duties as assigned.
EDUCATION, SKILLS & EXPERIENCE :
Bachelor’s degree in social work, human services, or related field OR equivalent combination of education and experience.Minimum 2-4 years' experience in human services / lived experience, counseling, residential setting, or the ability to be trained.Knowledge of domestic and sexual violence including elder abuse and ability to develop positive, compassionate relationships with victims / survivors through trauma informed approach.Knowledge of community-based service delivery systems, in particular knowledge of local housing systems such as Section 8, supportive and transitional living services.Ability to work in a crisis-oriented setting, provide trauma informed care, and knowledge of crisis intervention techniques.Strong interpersonal, oral communication and written skills.Proficient planning, organizational, time management, and administrative skills.Strong conflict resolution skills with both internal and external contacts.Must possess a valid New York State Driver’s License and have access to a reliable vehicle for travel throughout the area and to transport clients.Ability to assist clients in identifying, understanding and prioritizing problems and in obtaining information on available resources and assist in seeking help from intervention systems, including criminal justice, legal, medical counseling and income programs at various community partner locations.Adaptability to be flexible and change priorities as needed.Displayed ability to work both independently and collaboratively in a team environment.Strong training and facilitation skills.Proficient in Microsoft Office Suite.Completion of a satisfactory background check.SALARY RANGE
$20.00-$23.00 hourly, non-exempt
