  • Tuition Assistance\n\nResponsibilities\n\nAs a Home Health Registered Nurse (RN), here's a big-picture view of what you'll do : \n
  • Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility.\n
  • Work with a team of physicians and oversee a multidisciplinary team providing patient care.\n
  • Examine patients, do assessments, document progress, and report changes to their physician.\n
  • Educate patients on their plan of care, prescribed medication, therapy, diet and exercise.\n
  • Coach family members on the patient's plan of care, medication, and home safety.\n
  • Ensure goals are met and coordinate discharge from services.\n\nRequired Skills\n\nA few must-haves for Home Health Registered Nurses (RN) : \n
  • Graduate of an accredited Registered Nurse (RN) program\n
  • Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license\n
