Social Worker / Case Manager
Thomasville, GA | $15.00 - $25.00 / Hour
A service center is looking for an Social Worker / Case manager to support individuals, foster independence, and overall quality of life. This role involves developing and monitoring Individual Service Plans, coordinating services and opportunities, advocating for individual rights, and ensuring compliance with all relevant standards and regulations. This person will collaborate closely with individuals, families, and interdisciplinary teams to create tailored strategies that promote meaningful daily activities, inclusion, and self determination
Responsibilities
- Develop, monitor, and revise Individual Service Plans(ISPs) that support meaningful daily activities and community engagement
- Conduct person centered assessments and facilitate planning meetings that include individuals, families, and interdisciplinary teams
- Coordinate access to day services, prevocational activities, supported employment, volunteerings, and other enrichment opportunities
- Support individuals in exercising informed choices and pursuing goals related to work, recreations, learning, and social interactions
- Ensure timely documentation of services, outcomes, and individual progress in alignment with DBHDD and Medicaid standards
- Advocate for individuals’ rights, inclusion, safety, and well-being across all environments.
- Collaborate with team members to address barriers to full participation and create individualized support strategies.
- Monitor service quality and satisfaction and respond to concerns or incidents in a timely and professional manner.
- Maintain confidentiality and ensure ethical standards are followed in every interaction.
- Participate in ongoing training, team meetings, and quality assurance activities.
Requirements
Bachelor’s degree in Social Work, Human Services, Psychology, or related field1-2 years of experience in the field of intellectual and developmental disabilities, ideally in day or community based servicesCurrent CPR / First Aid certification and successful completion of DBHDD required trainingValid Georgia's driver’s license with access to reliable transportationCommitment to person centered thinking, advocacy, and inclusive community practicesKnowledge of DBHDD policies, HCBS Settings Rule, and Medicaid Waiver service expectationsExcellent communication, planning, and documentation skillsAbility to coordinate across multiple systems and providers to ensure holistic careCompassionate, resourceful, and adaptable in meeting diverse needs and goalsWilling to undergo criminal background check and fingerprintClean drug screenLocation : Thomasville, GA
Pay Rate : $15.00 - $25.00 / Hour
Schedule : Monday – Friday 8 : 00 am – 4 : 00 pm
