job_description.job_card.job_descriptionAssure ongoing interdisciplinary assessment of the patient. Ensure our home care clinicians meet and exceed standards for safe, effective and efficient clinical practice through support and oversight of our home care staff in accordance with all applicable federal and state laws, rules and regulations. Supervise paraprofessional and licensed professional team members. Use clinical decision making to efficiently and effectively manage an assigned patient population. Assure the development of the education for the patient and the family, and other caregivers to promote patient progress toward mutually established goals. Graduate of an accredited Physical Therapist Program and active PT license in Colorado Minimum of three (3) years of physical therapy experience in home healthcare preferred Two (2) years of management experience preferred Two (2) years of OASIS experience, preferred. Ability to conduct extensive OASIS documentation and chart audits. CPR Certification (demonstration course required) MUST be computer proficient. Able to work independently and provide effective communication to patient / family, team members, and other health care professionals. Knowledge of state and federal home health regulations
