  • Work / life balance\n\nWhat You'll Do : \n
  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).\n
  • Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation. Document such reports and observations in the clinical record and notify supervisor.\n
  • Regularly re-evaluates patient nursing needs.\n
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.\n
  • Uses health assessment data to determine nursing diagnosis.\n
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.\n
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the provider.\n
  • Provides direct patient care as defined in the State Nurse Practice Act.\n
  • Counsels the patient and family in meeting nursing and related needs.\n
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.\n
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.\n
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.\n
  • Prepares clinical notes and updates the primary provider when necessary and at least every 60 days.\n
  • Communicates with all relevant providers regarding the patient's needs and reports any changes in the patient's condition; obtains / receives orders as required.\n
  • Communicates with community health related persons to coordinate the care plan.\n
  • Performs other duties as assigned.\n\nQualifications : \n
  • Graduate from an accredited registered nursing program and licensed in the state\n
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)\n
  • Home health experience preferred\n
  • Possess and maintain valid CPR certification.\n
