Case Manager Responsibilities :
Must have knowledge of functionas of the facility nursing department and their capabilities to provide the client with the appropriate health care.Provide on-site clinical evaluation of the referred resident with prompt communication to the referral source .Develop care plans and coo rdinate clinical evaluations .Utilize regulatory agency guidelines and managed care concepts when reviewing clients for admission .Perform on-site evaluations incorporating the following :Contact with the Social Service DepartmentDevelop positive working relationships. Seek additional referrals .Inform and educate of facility's availability and services.Rev i ew of the resident's Medical RecordUtilize Clinical Review Process as established by the facility .Obtain financial and payment source information .Cl i ent visitIdentify reason for v i sit and the facility .Counsel client and / or family concerning available serv i ces and suitability of client ' s admission.Evaluate the client's degree of orientation and appropriateness of the therapy program.Establish the degree of support systems that can be utilized for a comprehensive discharge plan , i.e . family , housing, care takers , etc.Assess all client care needs including all equipment and number of nurs i ng hours.Professional contacts (physicians, case managers, therapist)Identify facility services.Discuss treatment plan and discharge goals of the referred client.Prov i de to the Admin is trator a list of clients assessed needs which includes cost facto r s such as equipment , infusion therapy, extensive dressing, continuous oxygen therapy, supplemental feedings , etc .Investigate and clearly define the i nsurance el i gibility coverage as related to the level ofForecast the duration of stay and the practicality of an assured discharge .Collaborate with the Subacute Manager to make clinical determ i nation on the nursing staff's ability to provide the required care of each resident admitted.Finalize and secure all necessary admission documentation , i.e . contracts , financial statements, letters of acceptance of fees from insurance companies or any other provider source agreements.When admission of client has been approved , inform nursing and other disciplines of the date of arrival.Collaborate with the Subacute Unit Manager to organ i ze and plan for special equipment and / or services which are required.Provide inservice to referral sources, agencies, organizations and profess i onals in relation to services, facility policy and procedures and knowledge of payment systems .Participate i n marketing management by collecting and organizing data on all c li ent referrals , admissions, denials and reason denied . Develop special marketing projects ut i lizing evaluations and surveys. Ide ntify positive and / or negative trends and report to the Administrator .Work as an interdisciplinary team member to keep staff informed of client's t herapy , medical , dietary and psychosocial needs as part of the care planning process . Act i vely participate in monitoring the client's progress to determine continued insurance eligibility .
