About The Position

Job Title : Home Health Registered Nurse

Profession : Registered Nurse

Specialty : Home Health

Duration : 20 weeks

Shift : Day

Hours per Shift : 8 hours (08 : 00 - 17 : 00)

Experience : Minimum of 1 year in home health with infusion experience and skilled home care experience.

License : Valid state RN license required.

Certifications : BCLS required.

Must-Have :

• Infusion experience.
• Skilled home care experience (not just private duty nursing).
• Home health experience with OASIS training and experience.
• IV skills and wound care expertise.
• Experience with high-risk diagnoses such as HF, COPD, MI, Pneumonia, Stroke, and post-operative care.

Description :

Provide nursing care to patients in their homes.

Conduct cardiopulmonary assessments and basic wound care.

Manage ABI compression dressing and wound vac dressing.

Provide Pleurx / Asept drain care and perform foley catheter changes and care.

Carry out staple / suture removal and use glucometer and pro-time machine.

Administer SQ / IM injections and perform lab draws via venipuncture.

Demonstrate IV skills, PICC line or midline care, and dressing change.

Access and deaccess SQ ports.

Complete skilled home care visits (6-8 per day).

Work day / shift hours may vary based on office needs.

Maintain proper documentation using specified electronic systems.

Manage visits and scheduling based on patient needs.

Must possess a valid driver's license and have reliable transportation for home visits.

COVID-19 vaccination is required or an exemption form must be on file.

Flu shot or declination is required, with mandatory mask-wearing while working.

Complete required modules prior to start and annually thereafter.

All home health RNs complete the same training modules.

Ensure compliance with all health and safety regulations.
