job_description.job_card.job_descriptionPerform initial nutrition screenings and determine patient nutrition risk level (low, moderate, high) upon admission. Conduct comprehensive nutrition assessments for high / moderate-risk patients within 24-48 hours of admission, and for all outpatients on their initial visit. Utilize the Nutrition Care Process to determine nutrition diagnoses, evaluate requirements, and develop, implement, and re-evaluate individualized care plans. Calculate patient nutrition requirements using standardized formulas (calories, protein, fluid). Initiate, modify, and manage diet orders to meet patients' calorie-protein requirements. Recommend appropriate enteral formulas, nutrition supplements, and initiate nutrition support consults as needed. Monitor patient nutrition status through meal rounds and calorie counts, evaluating the ongoing appropriateness of diet prescriptions. Perform timely nutrition reassessments and follow-ups based on patient risk level and established policy. Educate patients and their family members on nutrition principles and care plans. Coordinate discharge planning, including arranging for enteral feeding and home care services for outpatients. For outpatient clinics, coordinate the delivery of nutritional supplements, obtain prior authorizations (e.g., Medicaid), and submit necessary prescriptions. Communicate effectively with physicians, nurse practitioners, and the broader healthcare team regarding patient nutrition issues and recommendations. Participate in multidisciplinary rounds for designated clinics and provide cross-coverage for other outpatient areas as assigned. Refer patients to other services such as GI Nutrition, Social Services, and Speech Therapy as needed. Education : Bachelor's Degree in Nutrition & Dietetics. Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration. Certified Dietitian Nutritionist (CDN) in New York State within 12 months of employment (Not required for NJ positions). Proficient in written and verbal English communication. Strong verbal communication, listening, and customer relationship skills. Familiarity with Microsoft Office Suite (Word, Excel, PowerPoint). Demonstrated analytical, time management, and priority-setting skills. Excellent judgment and decision-making capabilities. Education : Master's Degree in a related field. Experience : Oncology experience.
