Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.

As a Centralized Intake Worker, you will be responsible for completing Centralized Intake interviews, assessments, and placement and referral activities in the PHC Homeless Support Services Department or in the community to assist with housing placement, case management, and locate income streams to prevent or respond to homelessness. Provides quality customer service, ensures accurate client data, and coordinates with team members to provide referrals to community agencies, emergency shelters, and housing programs. Demonstrates iCare values in daily work.

What You Will Do

Assists clients with housing related needs, providing excellent customer service.
Provides referrals to clients based on client eligibility for programs and / or mainstream resources.
Enters data into HMIS in accordance with Centralized Intake (CI) policies and procedures.
Follows all policies and procedures including but not limited to CI and PHC policies, all grantor policies or requirements; ensures compliance. Maintains confidentiality except in specific situations such as signed releases by the clients or in situations of actual or threatened harm by or to the client.
Answers and responds to clients via phone calls or in-person interactions in a timely manner; records information and enters client contact notes in HMIS accurately.

Qualifications You Need to Bring

Required :

Bachelor’s degree or equivalent combination of education and experience (if allowed by granting agency).

Strong customer service orientation with commitment to service excellence.

Excellent written and verbal communication skills.

Strong interpersonal skills and team orientation with ability to work collaboratively and build / maintain professional relationships at all levels with a diverse group of individuals.

Proficient using Microsoft Office applications and general computer skills.

Detail oriented with high degree of accuracy.

Organization and prioritization skills.

Demonstrates professionalism with ability to interact with others in an empathetic manner and maintain composure in all situations.

Licenses & Certifications : Valid Chauffeur’s Driver’s License; current Mandatory Reporter Training : Child and Dependent Adult Abuse recognized by the State of Iowa. All are to be obtained during introductory period if not current.

Preferred :

Bachelor’s degree in health care, psychology, counseling, mental health, human services, or related field. Continuing education in housing and homelessness.

Work experience with vulnerable populations, including people experiencing homelessness.

Community Health Center experience.

Demonstrated competency using HMIS.

Experience with homeless system in Polk County.

Knowledge of program terminology.

Ability to effectively use HMIS system to document all client contact.

Your experience and skills determine your base pay. The hiring range for this position is typically $17.00 - $21.25 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $25.50 per hour.
