The Case Management Specialist (CMS) provides comprehensive case management, resource navigation, and advocacy services for clients at the Women's Residential Center (WRC), ensuring access to stable housing, financial assistance, and long-term recovery supports. The CMS facilitates evidence-based case management, transition, and substance use disorder (SUD) groups while working collaboratively with a multidisciplinary team to deliver gender-responsive and culturally competent care. This role supports clients with complex needs, including co-occurring disorders, trauma histories, and involvement with the criminal legal or child welfare systems, promoting long-term stability and self-sufficiency.

Education, Training and Experience :

  • Bachelor's degree in human services or related field, with at least two years of experience in case management, resource navigation, and advocacy for at-risk populations. Additional relevant experience may substitute for education requirement.
  • CADC I certification required.
  • Required education, knowledge and skills to qualify as a Qualified Mental Health Associate (QMHA) in accordance with Oregon Administrative Rules is preferred.
  • Experience providing case management and / or resource information, referral and advocacy services for at-risk populations who are experiencing a combination of challenges including, chemical dependency, mental health, legal system involvement, and parenting issues.

Other Eligibility :

  • Must complete a criminal history background check and receive authorization from the State of Oregon's Background Check Unit to work in a Residential Treatment Facility.
  • Ability to drive to offsite locations using a personal or company vehicle. An active driver's license, current insurance, a reliable vehicle and ability to pass our insurer's driving standards.
  • Duties and Responsibilities :

  • Assesses client needs and develops individualized service plans that support housing, employment, and recovery goals, ensuring a successful transition to independent living.
  • Ensures all intake documentation, including immediate needs assessments and client chart updates, are completed in compliance with Oregon Administrative Rules.
  • Facilitates evidence-based transition and life skills groups, including Transition, Case Management, Ready to Rent and Women's Wellness group, to equip clients with essential tools for self-sufficiency and to develop skills that enhance long-term recovery and independent living.
  • Provides immediate support in crisis situations, assisting clients with urgent needs and connecting them to appropriate interventions to ensure clients receive timely assistance, reducing the likelihood of escalation or disengagement from services.
  • Connects clients with external resources such as housing assistance, legal aid, healthcare, and financial support to remove barriers to stability and gain access to critical services that promote economic security and well-being.
  • Works collaboratively with clinical counselors to align case management with clients' treatment goals, ensuring a coordinated and integrated approach to care.
  • Maintains therapeutic and supportive relationships with clients to foster trust, encourage long-term stability, and improve retention and successful program completion.
  • Provides gender-specific and culturally competent services to ensure individualized care that supports each client's unique recovery needs and lived experiences.
  • Develops and sustains positive relationships with community partners and referral sources to improve service coordination and expand resource options for clients.
  • Facilitates additional groups as needed to ensure clients have access to a variety of support options that enhance engagement in recovery and skill development.
  • Ensures all client records are maintained in compliance with Oregon Administrative Rules and HIPAA / 42-CFR Part 2 confidentiality standards to ensure that the program remains compliant with regulatory guidelines, ensuring privacy and quality of care.
  • Maintains professional boundaries with clients and co-workers to ensure ethical service delivery and a safe, supportive work and treatment environment.
  • Engages in program and organizational continuous improvement initiatives, including Client Proposal, Client Concern, and Grievance processes, to enhance service effectiveness and responsiveness to client needs.
  • Participates in ongoing professional development, including training on cultural competence, trauma-informed care, and best practices in case management to remain aligned with current best practices, enhancing overall program effectiveness.
  • Engages in staff meetings, case consultations, and supervision to ensure a collaborative team approach in supporting clients, strengthening service delivery and client outcomes.
  • Completes additional tasks as assigned by the supervisor or Program Management to support program operations and ensure client care and organizational goals are met efficiently.
  • Knowledge, skills and Abilities :

  • Knowledge of trauma-informed care to ensure services promote safety, empowerment, and resilience.
  • Knowledge of cultural, social, and economic factors influencing addiction and mental health to provide effective and responsive case management.
  • Knowledge of legal and ethical standards in case management, including confidentiality, mandated reporting, and informed consent to protect client rights and maintain compliance.
  • Knowledge of community resources and support services available for individuals seeking recovery, including self-help groups, detox facilities, and medical services to connect clients with essential supports.
  • Knowledge of co-occurring mental health and substance use disorders to provide comprehensive and integrated case management.
  • Knowledge of systemic barriers that impact housing, employment, and recovery for marginalized populations to strengthen advocacy efforts and improve service coordination.
  • Knowledge of federal and state policies related to housing, public assistance, and employment programs to provide accurate information and support client stability.
  • Skilled in handling crisis situations and providing immediate support and referrals for clients in acute distress to ensure client safety and prevent disengagement.
  • Skilled in facilitating evidence-based life skills and groups to enhance client self-sufficiency and long-term stability.
  • Skilled in conflict resolution and mediation to improve service coordination and address barriers to client progress.
  • Skilled in time management and organization skills, efficiently prioritizing tasks, allocating resources, and meeting deadlines to ensure timely service delivery and effective case management.
  • Skilled in maintaining accurate and confidential client records and case management documentation, adhering to accepted standards and legal and ethical requirements to ensure compliance with regulatory standards and protect client privacy.
  • Skilled in using electronic medical records and documentation systems to enhance efficiency and service tracking.
  • Skilled in advocating for client rights and needs within systems of care to ensure access to fair and appropriate services.
  • Skilled in navigating insurance and public assistance systems to support client access to care to connect clients with eligible benefits and resources.
  • Ability to approach individuals with empathy, active listening, and a non-judgmental attitude, demonstrating respect for their unique experiences and strengths to create a supportive and non-judgmental space for client engagement.

  • Supervisory Responsibility : The Case Management Specialist does not supervise other employees.

  • Environmental Conditions : Work is performed in a clinical treatment center with moderate light and sound. May be required to intervene in crisis situations or work with agitated clients. Travel locally to events and meetings on a regular basis.

    Expected Hours of Work : Monday through Friday, 40 hours per week. Additional hours as needed.
