A Brief Overview
Responsible for accepting and facilitating new patient referrals. Exemplary communication and customer service with referring physicians, patients, department providers and staff required. Critical thinking related to pre-visit tests and scheduling parameters. Schedules all pre-visit tests and ensures the patient is fully informed of the scheduled tests and provider visit. Must be able to multi-task efficiently while working in a fast-paced, interruptive environment and have the ability to adapt to change quickly.
What You Will Do
- Answers patient referrals promptly within 24 hours. Communicates with referring provider offices and new patients including new patient welcome calls. Keeps patients informed about status of their appointment, if testing is needed prior to appointment, and obtains the medical records required for the appointment.
- Ensures all critical information needed from the referring physician is obtained and appropriately routes referral to physician and care team based on diagnosis and availability of appointment time.
- Assesses the referral to determine the necessary pre-visit tests needed prior to the patient first visit with the guidance of the provider and nursing teams. Initiates the scheduling of pre-visit testing to meet the needs of the patient and provider while ensuring a timely first visit for the patient.
- Provides communication with the patient throughout the process related to upcoming appointments, testing and instructions if applicable. Reviews with patient as needed to ensure understanding.
- Communicates with co-workers promptly when additional action or follow-up needs to be taken for orders to be completed, e.g. requesting pre-authorizations, dictation, pre-medications and lab results.
- Receives verbal and written orders from provider and assures the completion of provider orders.
- Provides general patient care, rooms patients and does assessments, and provides educational materials to patients working under the supervision of a provider.
- Maintains and views patient records, charts, and other pertinent information to include, but not limited to : works up new patient charts; reviews charts and assures that all necessary information is in the chart; reviews all lab, hospital and other reports; and updates chart as necessary. May notify patients of abnormal tests and treatment as directed by the provider.
- Assists with management of provider schedules as needed.
