Your Responsibilities as an Intake Nurse

In this vital role, you will :

Serve as a liaison between referral sources, health professionals, and our field staff

Coordinate incoming calls, providing essential information to patients and referral sources

Communicate with patients and families to arrange care services

Conduct intake screenings and document necessary information for admission decisions

Log referrals and prepare reports, ensuring timely responses to inquiries

Promote our care services to prospective patients

Receive verbal orders from physicians to initiate care, where permitted by law

Enter customer information into our management system

Essential Requirements

To be successful in this role, you should have :

A degree from an accredited nursing school

Current RN licensure in the state of Michigan

At least one year of experience as a Registered Nurse

Prior home health care experience is preferred
