• Offering Pay-per-Visit Rates - $35.00 per point - $27.00 base rate
• Flexible scheduling, comprehensive benefits package and benefit eligibility upon start date

Overview

As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.
