Current and valid RN license in the state of practice Minimum of 1 year of RN experience BLS certification (or obtained within 90 days of hire) Reliable transportation, valid driver's license, and minimum required liability auto insurance Clearance of background check, including immunization and communicable disease requirements Bachelor of Science in Nursing (BSN) preferred Experience with older adult or medically complex populations preferred Background in home care, long-term care, or managed care environments Strong clinical assessment, care coordination, and patient education skills Familiarity with electronic health records and healthcare software Team-oriented mindset with excellent communication and problem-solving abilities Passion for delivering exceptional care to vulnerable populations in diverse settings

Role Summary
Seeking a compassionate and experienced Registered Nurse Care Manager to join their
dedicated care team. This dynamic role involves assessing patient needs, delivering skilled nursing care,
and developing individualized care plans to promote health and quality of life for an aging population. The
RN Care Manager will collaborate closely with a multidisciplinary care team to coordinate and deliver care
across clinical, home, and community settings.
