Job Description & Requirements Specialty : Home Health Discipline : RN Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Travel Shift : 08 : 00-17 : 00 NURSYS EMR : Homecare Homebase (HCHB) Experience Required : 2 years (will consider 18 months under highest level of professional license, must have 1 year experience in field of specialty) Role Summary : The Home Health Visit Nurse works collaboratively with the Home Health RN Care Manager and other disciplines, providing skilled care, education, treatment, and emotional support to patients. Monitors and assesses care while ensuring the patient attains and maintains their highest level of functioning. Ensures that the Physician and care team are aware of pertinent changes in patient condition and progress toward goals. Collaborates with Home Health RN Care Manager to ensure the plan of care is appropriate and implemented / modified based on patient responses.
